Canmore Pharmacy ("we", "us", "our", "the Pharmacy") is committed to protecting the privacy and confidentiality of your personal and health information. This Privacy Policy explains how we collect, use, disclose, and safeguard information about you, whether you are a patient receiving care in our pharmacy or a visitor to our website at canmorepharmacy.ca.
As a licensed pharmacy operating in Alberta, we are a "custodian" of health information under Alberta's Health Information Act (HIA), and we are also subject to Alberta's Personal Information Protection Act (PIPA) for personal information that is not health information (such as employee or general business contact information). We also follow the professional practice standards and code of ethics of the Alberta College of Pharmacy (ACP) regarding confidentiality of patient information.
1. Information We Collect
Depending on your relationship with us, we may collect:
- Health information, including your name, date of birth, personal health number, prescription and medication history, allergies, medical conditions, immunization records, and information shared with our pharmacists during consultations or clinical services.
- Contact information, such as your name, address, phone number, and email address.
- Payment and billing information needed to process prescriptions and insurance claims (we do not store full credit card numbers).
- Website and communication information, such as information you submit through our "Transfer Prescription" or "Send Us a Message" contact forms, and standard technical information (like browser type and IP address) collected automatically when you visit our site.
2. How We Collect Information
We collect information directly from you (in person, by phone, fax, or through our website forms), from your physician or other treating health care providers, from other pharmacies when you transfer a prescription to us, and from Alberta Netcare / the provincial Electronic Health Record where authorized by law, in order to provide you with safe and effective pharmacy care.
3. Why We Collect, Use, and Disclose Your Information
We collect, use, and disclose your personal health information only for purposes permitted or required by law, including to:
- Dispense and manage your prescriptions safely, including checking for drug interactions and allergies;
- Provide clinical services such as medication reviews, prescribing, and immunizations;
- Communicate with your physician, nurse practitioner, or other members of your care team;
- Process billing and claims with Alberta Health, Alberta Aids to Daily Living (AADL), and private insurers;
- Contact you regarding prescription refills, pickups, or important safety information (such as drug recalls);
- Meet our legal and professional obligations to the Alberta College of Pharmacy, Alberta Health, and other regulatory bodies; and
- Respond to inquiries submitted through our website contact and prescription transfer forms.
We do not sell, rent, or trade your personal or health information to third parties for marketing purposes. Your information is disclosed only to those involved in your circle of care, as required by law, or with your consent.
4. Website Forms and Cookies
Information you submit through our "Transfer Prescription" or "Send Us a Message" forms is sent securely to our pharmacy team and used only to respond to your request. Please avoid submitting sensitive medical details through these forms — our staff will follow up with you directly, by phone, for anything requiring detailed health information.
Our website may use basic cookies and similar technologies to remember your preferences and understand general site usage (such as which pages are visited most). This information is aggregated and is not used to identify you personally. You can disable cookies through your browser settings at any time.
5. How We Protect Your Information
We maintain physical, administrative, and technical safeguards appropriate to the sensitivity of your information, including secure pharmacy management software, restricted staff access on a need-to-know basis, staff training on confidentiality, and secure destruction of records that are no longer required. Our website uses standard encryption (HTTPS) to protect data submitted through our forms.
6. How Long We Retain Your Information
We retain patient health records for the period required by the Alberta College of Pharmacy's standards of practice and applicable law, currently a minimum of ten (10) years from the date of the last patient interaction (longer for minors). Website form submissions are retained only as long as needed to respond to your inquiry.
7. Your Rights
Under the Health Information Act and PIPA, you have the right to:
- Access your personal health and personal information held by us;
- Request correction of any information you believe is inaccurate or incomplete;
- Withdraw or restrict consent for certain uses or disclosures of your information, subject to legal and professional limitations; and

To exercise any of these rights, please contact our Privacy Officer using the information below. We may require you to verify your identity before providing access to your records, and a reasonable fee may apply for large or complex requests as permitted by law.
